Applicant Requirements
- Application - Livery Driver
- Current Ontario Class "G" Driver's Licence
- Proof of Canadian Citizenship, Landed Immigrant Status or government-issued Work Permit
- Current colour photograph of applicant - passport sized
- Original Police Enhanced Screening Records Check dated no more than 30 days prior to date of application (must attend the licensing office to complete required documents to obtain the police clearance).
- Statement of driving record issued by Ministry of Transportation Safety and Regulations Division dated no more than 30 days prior to date of application.
- Photo I.D. card will be issued (every 2 years)

Applicant Requirements:
- Application-Livery Plateholder
- Business Ownership Declaration
- Rate Schedule
- Copy of vehicle ownership
- Valid Standard Safety Certificate dated no more than 30 days prior to the date of application
- Certificate of insurance - $3,000,000 minimum liability
- Proof that applicant is over 18 years of age
- Market Study (see schedule 3-section 4 of By-law 137-2007 on By-laws Online)
- If a corporation, the applicant shall file copies of the article of incorporation.
- If a partnership, the applicant shall provide the I.D. and address of each member of the partnerhsip. See section 4.2 (iii) of By-law 137-2007 on By-laws Online.

New applicants must be submitted to the Windsor Licensing Commission for approval, (see By-law 137-2007 on By-laws Online for required information).
